A simple 5S audit tool in a spreadsheet (Excel).

The scores are transferred to a scoreboard to see the development.

The audit form has 6 measuring parameters.

The 6 measuring parameters

1. Safety. 
2. Physical work places.
3. Floor.
4. Rack and shelves.
5. PC, instructions and boards.
6. Cleaning materials and standards.

The measuring parameters can easily be changed.

5S audit procedure

The sequence of a 5S audit:

1. Print a blank "Audit form" or bring a laptop at the audit.
2. A 5S patrol audits all involved departments. 
3. Scores are entered onto the sheet "Registration form". The overall diagram is automatically updated and can be printed.
4. Local scores and diagrams for each department can also be printed.
5. A date for the next audit is agreed.
